A 345mm Rear Brake Disc for the Land Cruiser 200 series as listed in Specs and Fitment from Brake World - still in original box.
This 5 stud hole, 18mm thick rear disc is one of the largest Toyota rear discs, it is also machined on the inside to exacting tolerances to take the rear handbrake shoes.

Use Rear pads;
PAD012 or PAD012G for Genuine.
